Forms Required for New and Returning Scouts
Note: These forms must be completed at Pack registration by all New Cub Scouts.
Note: In addition, these forms must be completed annually at Pack registration by All Cub Scouts.

Online BSA Application (Pack 518) - for youth or adults who have not
yet registered with as Scouts
- BSA Youth Application (PDF) - for youth who have not yet registered with as Scouts
- BSA Adult Application (PDF) - for adults who wish to become adult leaders and require registration as Scouters
- BSA Member Transfer Form - for Scouts or Scouters who wish to transfer an active BSA membership
Please bring completed forms and any required fees to one of Pack 518’s monthly Pack meetings, usually held the first Monday of each month, starting at 7:00 PM in Suwanee Elementary School.
